Card Replacement Request
To request a replacement certification card, simply fill out the information below. We will research your request as soon as possible. Remember, a certification card is valid for only 24 months after certification. Once we find your record, we will send you the card, email you confirmation, and charge your bankcard the $10 replacement fee.
In order for us to replace your lost or stolen card, you must have taken your course through Rescue Training Institute. Please provide us the name you were registered under when you took the course.
Should we not be able to locate you in our system, we will simply email you our findings. We will not charge your bankcard.
